1. RECAP-369 - Added table REQUEST_INST_BIB_T

2. RECAP-471 - Dropped column EMAIL_ID from table REQUEST_ITEM_T, Added column NOTES to the table REQUEST_ITEM_T, Modified the insert statement for table REQUEST_ITEM_STATUS_T.

3. RECAP-479 - Increased PERMISSION_NAME column size in table PERMISSION_T and updated the permission name and roles name.

4) Solr Changes:

          NIL

UAT is a culmination of all tasks that were part of the previous UAT release and SCSB versions 0.9.7, 0.9.8 and 0.9.9.
